The medical staff can't resus you if there is a current AHD available that requests no resus.

Don't forget to add that you don't want any treatment of any sort either, if you are found alive but unwell. Perhaps you might consider No antibiotics or other medications that may prolong your life, but that you want to be made comfortable with pain relief medications?
