When medical interventions pose unnecessary, disproportionate risks of harm, healthcare providers should refuse to offer them—even when requested.

More boring evidence of the death spiral of the West; totally lost in space.

Dear Maryanne (the author),

.

My ignorance of gender dysphoria knows no bounds. I find your article on paediatric gender dysphoria interesting, informative and pleasant to read.

An aspect that I am particularly interested in is the cause(s) of paediatric gender dysphoria.

I see that the US population in 2021 was 336,997,624 (and now stands at about 342 million). As there were 42,167 diagnosed patients aged 6–17 in the US in 2021 that represents only 0.0125% of the American population.

If we apply the same rate to the US population in 2025 of 342 million, we arrive at a total of 42,750 patients.

Are these children the result of manufacturing errors of nature ? What causes them ? What went wrong ? Is it the result of genetic inheritance, faulty programming, procedural error, poor quality biological material. Is it purely accidental or fortuitous … ?

If nothing went wrong in the natural process, does this mean that there are various versions of human beings, various gender combinations, other than simply male and female ?

If it is part of the normal process, why are there so few of them ?

The average life expectancy of all human beings on earth is 73.52 years in 2025 and is projected to increase to 81.7342 in the year 2100. I wonder how these various versions of mankind compare with each other in terms of longevity and resilience.

I should be grateful if you could throw some light on any of these aspects of this fascinating subject.

Thank you Maryanne, this is an important and judicious article, and I hope somebody takes a bit of notice.

50 years ago, trans was barely understood, medical treatment very difficult to get, and successful sex-changers (unless they were actual performers like Carlotta) would be well advised to disappear into the woodwork, as did the brilliant computer scientist Lynn Conway.

Now it's gone too far the other way, trans is applauded as some kind of mystical state of wisdom, which is nuts. Hence the stupidity, of letting trans-women blitz women's sports. We should be very cautious about strong interventions before legal adulthood - when it's your call, your risk.
