The Lancet has always been an idiosycratic journal and, traditionally, has not used a peer review system rather, editorial decisions have been taken "in–house". This of course is in clear distinction with its old rival the British Medical Journal and in "my time" Lancet was always regarded as a "bit suss". To compare Wakefield with Semmelweiss BTW is absolutely ridiculous.
